Chronic health conditions

What is a chronic condition or disease?

Chronic diseases  are long lasting conditions that have ongoing effects. There are numerous conditions and the  Australian Institute of Health and Welfare (AIHW) list 10 of the most common that are the cause of significant health problems.

  • asthma
  • arthritis
  • back pain
  • cancer
  • chronic kidney disease
  • cardiovascular disease
  • chronic obstructiv pulmonary disease (COPD)
  • diabetes
  • osteoporosis
  • selected heart disease
  • Stroke
  • vascular disease
  •  mental and behavioural conditions (including mood disorders, alcohol and drug problems and dementia)

There are of course ,numerous others conditions and the AIHW say that many people with chronic disease can have more than two conditions and this can make management difficult.

Chronic conditions can worsen over time becoming more serious which can lower quality of life contibuting to long term disability and early death.

We can help you manage your conditions taking into consideration all aspects of each condition and planning with you to manage well in the long term.
